This really helped. I would have definitely missed some points for format if you didn't post this. i also tried googling it but i wasn't sure of the validity of some of the sources that same up. thanks.

this was pretty handy for me, it was great to have a format to look at in order to do it right;sometimes the Hacker book can be confusing.
